Participants are asked to update their name, local mailing address, email, phone number and emergency contact information in the Urban Teachers Participant Portal every August. Updated contact information and school placement information will also be requested each fall through a form.
To ensure that important mailings are always received and that the correct name is on all official records, Urban Teachers asks that participants notify their site team Data and Assessment Coordinator or Data and Certification Officer in writing within one (1) week of any changes in name or address and complete and sign a revised IRS W-9 form.
Participants are also asked to update their personal contact information records with the university partner.
